dc.description.abstract[SPA] La muerte por sumersión es un problema de salud pública importante. Es considerada como la segunda causa de muerte no intencional, tras las producidas por accidentes de tráfico y representa más de medio millón de muertes anuales en todo el mundo. El diagnóstico post mortem de ahogamiento continúa siendo uno de los más difíciles en la patología forense y con más frecuencia necesita exámenes complementarios. La determinación de elementos traza en sangre y humor vítreo puede ser aplicada en medicina forense para el diagnóstico de la causa de la muerte. En este sentido, uno de los principios generales que puede ayudar a confirmar el diagnóstico de sumersión es la presencia en la circulación sanguínea de sustancias químicas presentes en el agua de sumersión que se encuentran en el cuerpo humano en concentraciones muy bajas (elementos traza). El propósito de esta investigación fue analizar distintos elementos químicos en sangre y humor vítreo en distintas causas de muerte y su posible aplicación al diagnóstico de sumersión. Así, 19 elementos químicos (Al, As, Ba, B, Br, Ca, Cd, Cu, Fe, K, Li, Mg, Mn, Mo, Ni, Pb, Se, Sr y Zn) se determinaron en la sangre periférica, sangre del ventrículo derecho, sangre del ventrículo izquierdo y humor vítreo, procedentes de 68 cadáveres con diferentes causas de muerte: sumersión en agua de mar (n=41), sumersión en agua agua dulce (n= 14) y no sumersión (n=13) y el medio de sumersión, por espectrometría de masas con plasma acoplado inductivamente (ICP-MS). Los objetivos del estudio fueron: (1) Determinar el tratamiento más adecuado para aplicar a las muestras y estudiar la idoneidad de las técnicas de análisis elemental ICP-MS e ICP-OES. (2) Analizar la existencia de diferencias en las concentraciones de los elementos químicos entre los grupos diagnósticos y diferencias en las concentraciones de los elementos asociadas al sexo y a la edad de los fallecidos. (3) Determinar la existencia de correlaciones entre las concentraciones de los elementos bioquímicos analizados con el intervalo postmortem y correlaciones entre los elementos químicos en sangre y humor vítreo y el medio de sumersión. (4) Analizar la existencia de patrones definidos en los tres grupos diagnósticos y relaciones con los elementos químicos analizados. (5) Evaluar la capacidad discriminatoria de los elementos traza de forma individual y conjunta para diagnosticar las muertes por sumersión. (6) Análisis de ratios entre la concentración de los elementos químicos en agua de mar y humor vítreo y entre agua de mar y sangre. (7) Establecer un protocolo en el uso de los marcadores bioquímicos como herramienta complementaria en el correcto diagnóstico. Las conclusiones del estudio fueron las siguientes: (1) ICP-MS es la técnica de elección para el análisis de trazas y ultratrazas en matrices clínicas. (2) Nuestros análisis muestran que las concentraciones de Sr, Mg, B y Ca en las tres localizaciones sanguíneas son marcadores útiles en el diagnóstico de la sumersión en agua salada. La utilidad de las concentraciones de litio debe realizarse con precaución debido a sus bajas concentraciones en este medio. La determinación conjunta de estroncio, magnesio, calcio, boro y litio permiten clasificar al 83% de los casos de sumersión en agua salada. (3) El bromo presenta alta capacidad discriminatoria para la identificación de la sumersión en agua salada. (4) El análisis de hierro en sangre puede ser útil para el diagnóstico de la muerte por sumersión en agua dulce. La determinación conjunta de cinc, bario, estroncio y magnesio permite mejorar la capacidad de discriminación para identificar los casos de sumersión en agua dulce. (5) El análisis de componentes principales pone de manifiesto mayores niveles de Sr, Mg, B, Ca y Br en sangre del ventrículo izquierdo en sumersión en agua salada, lo que explica la aspiración de agua hipertónica, y mayores niveles de As, Se, K, Zn, Mn, Pb y Cu en sangre del ventrículo derecho, lo que evidencia la hemodilución. (6) El análisis de las aguas de sumersión muestra mayores concentraciones de Sr, Ca, B, Br y Mg en agua salada, lo cual se corresponde con los análisis de sangre de sumersión en agua salada y con las ratios agua salada/sangre. (7) Los análisis realizados en humor vítreo muestran por primera vez la utilidad de este fluido biológico para el diagnóstico de sumersión y se ha comprobado la utilidad del análisis de las concentraciones de Mg, Sr, Br y B en el diagnóstico de sumersión en agua salada. (8) La determinación de elementos traza en sangre y en humor vítreo es una buena herramienta complementaria para el diagnóstico de sumersión. [ENG] Drowning is a major public health problem. It is considered to be the second leading cause of death from unintentional injury, after road traffic injuries, and accounts for more than half a million deaths annually worldwide. The post-mortem diagnosis of drowning continues to be one of the most difficult in forensic pathology and it needs increasingly more frequent complementary examinations. Trace element determination in blood and vitreous humor can be applied in forensic medicine to diagnose the cause of death. One of the general principles that can help confirm the diagnosis of drowning is the presence of chemicals substances in the blood circulation, present in the drowning water that are found in the human body in very low concentrations (trace elements). The purpose of this research was to analyze different chemical elements in blood and vitreous humor in different causes of death and its possible application to the diagnosis of drowning. In this study 19 elements (Al, As, Ba, B, Br, Ca, Cd, Cu, Fe, K, Li, Mg, Mn, Mo, Ni, Pb, Se, Sr and Zn) were determined in peripheral blood, right ventricle and left ventricle blood and vitreous humor from 68 cadavers with different causes of death: seawater drowning (n=41), freshwater drowning (n=14) and no drowning (n=13), and drowning medium by inductively coupled plasma-mass spectrometry (ICP-MS). The study objectives were: (1) To determine the most appropriate treatment to apply to the samples and to study the suitability of the ICP-MS and ICP-OES elemental analysis techniques. (2) Analyze the existence of differences in the concentrations of the chemical elements between the different diagnostic groups and differences in the concentrations of the elements associated with sex and the age of the deceased. (3) To determine the existence of correlations between the concentrations of the biochemical elements analyzed with the postmortem interval and correlations between the chemical elements in blood and vitreous humor and the drowning water. (4) To analyze the existence of defined patterns within the three diagnostic groups and the relationships between the analyzed chemical elements. (5) To evaluate the discriminative power of the trace elements individually and jointly in order to diagnose drowning deaths. (6) Analysis of ratios between the concentration of chemical elements in seawater and vitreous humor and between sea water and blood. (7) To establish a protocol in the use of biochemical markers as a complementary tool in the correct diagnosis. The conclusions of the study were: (1) ICP-MS is the technique of choice for trace and ultratrace analysis in clinical matrices. (2) Our analyses show that concentrations of Sr, Mg, B and Ca in the three blood locations are useful markers in the diagnosis of seawater drowning. The usefulness of lithium concentrations should be made with caution due to their low concentrations in this environment. The joint determination of strontium, magnesium, calcium, boron and lithium allows to classify 83% of the cases of drowning in seawater. (3) Bromine has a high discriminative power in the identification of seawater drowning. (4) Blood iron analysis may be useful for the diagnosis of freshwater drowning. The joint determination of zinc, barium, strontium and magnesium allows to improve the power of discrimination to identify cases of freshwater drowning. (5) Principal components analysis shows higher levels of Sr, Mg, B, Ca and Br in left ventricular blood in sea water drowning, which explains the aspiration of hypertonic water, and higher levels of As, Se, K, Zn, Mn, Pb and Cu in the right ventricle blood, demonstrating hemodilution. (6) The analysis of drowning waters shows higher concentrations of Sr, Ca, B, Br and Mg in sea water, which matches the seawater drowning blood tests and the seawater / blood ratio. (7) The analyzes performed in vitreous humor show for the first time the usefulness of this biological fluid for the diagnosis of drowning and the usefulness of the analysis of the concentrations of Mg, Sr, Br and B for the diagnosis of drowning in sea water has been verified.
